Document: P107708

Dimensions: 1998 x 3012

Description: Page: 7 Photo 2: (Johnson 08/05) Pre-project Condition Stranded intake pipe visible middle of image post Spring High water damage. Staking upstream from intake marks project site for placement of new diversion headgate.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1504 x 1000

Description: Page: 7 Photo 3: (Johnson 11/05/05) Stripping forms from new diversion headgate structure. Forms are resting on coffer dam creating a walkable work area. BOR on-site staff inspects concrete finish prior to replacement of rock rip rap.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1501 x 996

Description: Page: 8 Photo 4: (Johnson 10/05) Site constraints required transfer of concrete from delivery truck to work site by excavator bucket.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1024 x 768

Description: Page: 8 Photo 5: (Johnson 10/05) Placement of concrete into headgate forms was completed by hand with site constructed funnels. All concrete work was mechanically vibrated after placement to ensure a uniform pour would be achieved given the small batch placement. Site constraints would not allow use of a pump truck.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1504 x 1000

Description: Page: 9 Photo 6: Johnson 08/05). Existing Dike Access Road prior to construction. Narrow width and unknown construction condition precluded use of the road for pump trucks.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1502 x 998

Description: Page: 9 Photo 7: (Johnson 11/05) Access road following completion of project. Surface was renewed and bladed as disposal site for waste material from headgate excavation. New dike surface provides improved access to the headgate for project maintenance. Note: Lack of tree/vegetation removal adjacent to dike access road.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1504 x 1000

Project(s): 2005-009-00

Document: P107708

Dimensions: 1492 x 998

Description: Page: 10 Photo 9: (Johnson 08/07) Newly constructed connecting channel between Ponds 2 and 3 following removal of culvert. Excellent passage now exists for all TES species. Note: Net pen visible in pond in use by WDFW for steelhead pit tag project.

Project(s): 2005-009-00

Document: P107708

Dimensions: 1504 x 1000

Description: Page: 11 Photo 10: (Johnson 08/06) Newly created side channel between Ponds 1 and 2 allows passage at all flows. A 28 foot section of 36” Dia. CMP provides vehicle access to the site. The Culvert was filled with cobbles following placement by means of a conveyor to ensure passage and bed roughness.
